(collectively Upper Deck), permission to email your child, enter your child in various
promotions and sweepstakes at your child’s request, and to obtain personal information
regarding your children. This information may include child’s name, address, birth
date, age, email address and parent or legal guardian’s name. Upper Deck respects
the privacy rights of our online visitors and recognizes the importance of protecting
and safeguarding the information collected about our underage customers. To assure

information so that in no event will information about our customers be made available

Upper Deck Sponsored Events
Your child's participation in an Upper Deck sanctioned tournament is voluntary.
By signing this consent form, you authorize the participation by your child in all
Upper Deck sponsored events. You understand that participation may result in your
child's name and city or state being made public on the Upper Deck website. You
also understand and agree that Upper Deck may contact your child to obtain information
pertinent to the tournament such as name, address, age and play level as well as
to send promotional items and other Upper Deck information. At any time, even after
consent has been given, you have the option to contact Upper Deck's Privacy Policy
Administrator at www.upperdeck.com and review,
alter or delete information collected about your child or to opt out of the collection
of data on your child, further participation from all Upper Deck sponsored events,
and/or contact or submission of promotional materials from Upper Deck to your child.
